Between one and five per 10, patient courses with penicillin result in allergic reactions, with one in 50, to one in , courses having a fatal outcome, accounting for 75 percent of anaphylactic deaths in the United States. Latex allergy has become a significant problem since the widespread adoption of universal precautions against infection.

Eight to 17 percent of health care workers experience some form of allergic reaction to latex, although not all of these reactions are anaphylaxis. Latex is in gloves, catheters, and countless other medical supplies, as well as thousands of consumer products.

Persons allergic to latex also may be sensitive to fruits such as bananas, kiwis, pears, pineapples, grapes, and papayas. Allergy extracts, antilymphocyte and antithymocyte globulins, antitoxins, carboplatin Paraplatin , corticotropin H. Acthar , dextran, folic acid, insulin, iron dextran, mannitol Osmitrol , methotrexate, methylprednisolone Depo-Medrol , opiates, parathormone, progesterone Progestasert , protamine sulfate, streptokinase Streptase , succinylcholine Anectine , thiopental Pentothal , trypsin, chymotrypsin, vaccines.

Cryoprecipitate, immune globulin, plasma, whole blood. Cold temperatures, exercise. Finally, radiographic contrast media can result in severe adverse reactions at a rate of 0. When history of exposure to an offending agent is elicited, the diagnosis of anaphylaxis is often obvious. Cutaneous manifestations of urticaria, itching, and angioedema assist in the diagnosis by suggesting an allergic reaction.

However, when gastrointestinal symptoms predominate or cardiopulmonary collapse makes obtaining a history impossible, anaphylaxis may be confused with other entities. If the diagnosis of anaphylaxis is not clear, laboratory evaluation can include plasma histamine levels, which rise as soon as five to 10 minutes after onset but remain elevated for only 30 to 60 minutes.

Urinary histamine levels remain elevated somewhat longer. Alternatively, serum tryptase levels peak 60 to 90 minutes after onset of anaphylaxis and remain elevated for up to five hours. Some patients have isolated abnormal tryptase or histamine levels without the other.

The initial management of anaphylaxis includes a focused examination, procurement of a stable airway and intravenous access, and administration of epinephrine. Examination may reveal urticaria, angioedema, wheezing, or laryngeal edema.

If the antigen was injected e. Epinephrine , dilution, 0. The site may be gently massaged to facilitate absorption. The dose may be repeated two or three times at 10 to 15 minutes intervals.

If severe hypotension is present, epinephrine may be given as a continuous intravenous infusion. Patients receiving intravenous epinephrine require cardiac monitoring because of potential arrhythmias and ischemia. If an intravenous line cannot be established, the intramuscular dose can be injected into the posterior one third of the sublingual area, or the intravenous dose may be injected into an endotracheal tube.

The patient should be placed supine or in Trendelenburg's position. Supplemental oxygen may be administered. Intravenous access should be obtained for fluid resuscitation, because large volumes of fluids may be required to treat hypotension caused by increased vascular permeability and vasodilation. While volume replacement is central to management of hypotension in anaphylaxis, other pressors such as dopamine Intropin , 2 to 20 mcg per kg per minute, may be required.

The report also highlights the importance of patient and family education about the recognition and management of anaphylaxis in the community. Clinical criteria for anaphylaxis have been proposed and validated. These clinical criteria for the diagnosis of anaphylaxis have been validated in emergency department studies in children, teenagers, and adults.

They have high sensitivity Foods, especially peanut, tree nuts, milk, eggs, crustacean shellfish, and finned fish, are by far the most common triggers of anaphylaxis in the pediatric population. Cofactors that lower the threshold at which triggers can cause anaphylaxis include exercise, upper respiratory tract infections, fever, ingestion of nonsteroidal antiinflammatory drugs or ethanol, emotional stress, and perimenstrual status.

Epinephrine is the medication of choice for the first-aid treatment of anaphylaxis. Through vasoconstrictor effects, it prevents or decreases upper airway mucosal edema laryngeal edema , hypotension, and shock.

In addition, it has important bronchodilator effects and cardiac inotropic and chronotropic effects. Delayed epinephrine administration in anaphylaxis is associated with an increased risk of hospitalization 22 and poor outcomes, including hypoxic-ischemic encephalopathy and death.

Epinephrine can be life-saving when injected promptly by the intramuscular IM route in the mid-outer thigh vastus lateralis muscle as soon as anaphylaxis is recognized Table 1. Epinephrine autoinjectors EAs can be used in health care settings to deliver a 0.

Also, symptoms can differ among patients, and even in the same patient from 1 episode to the next. Typically, more than 1 body organ system is involved. If the response to the first epinephrine injection is inadequate, it can be repeated once or twice at 5- to minute intervals. Subsequent doses are typically given by a health care professional along with other interventions. The need for subsequent injections did not correlate with obesity or overweight status.

Subsequent epinephrine doses are needed for severe or rapidly progressive anaphylaxis and for failure to respond to the initial injection because of delayed injection of the initial dose, inadequate initial dose, or administration through a suboptimal route. Food-induced anaphylaxis is associated with biphasic anaphylaxis less often than is venom- or drug-induced anaphylaxis. Reluctance to inject epinephrine promptly at the onset of anaphylaxis symptoms is best overcome by awareness that the severity of an anaphylactic episode can differ from 1 patient to another and in the same patient from 1 episode to another.

These effects cannot be dissociated from the beneficial effects of epinephrine. Epinephrine given by IM injection achieves peak concentrations faster than that given by subcutaneous injection.

There is no absolute contraindication to epinephrine treatment in anaphylaxis. Only 2 premeasured, fixed doses of epinephrine, 0. These doses are optimal for many children but not necessarily for all children. The 0. However, dose preparation can take laypersons as long as 3 to 4 minutes; moreover, doses typically are inaccurate and can sometimes contain no epinephrine at all when the solution is ejected from the syringe along with the air.

After consideration of the aforementioned alternatives that potentially lead to delay in dosing, incorrect dosing, or no dose at all and consideration of the favorable benefit-to-risk ratio of epinephrine in young patients with anaphylaxis, many physicians recommend the use of the 0.

On the basis of a pharmacokinetic study 40 and expert consensus, it is appropriate to switch most children from the 0. Most anaphylaxis deaths occur in community settings rather than in health care settings 1 , 16 , — 18 ; yet, some physicians fail to prescribe EAs for their patients at risk of anaphylaxis in the community. In fact, some experts have suggested that consideration be given to prescribing EAs for all patients with immunoglobulin E—mediated food allergy, because it is difficult or impossible to predict the occurrence or severity of future reactions.

Guidelines recommend prompt epinephrine injection for the sudden onset of any anaphylaxis symptoms after exposure to an allergen that previously caused anaphylaxis in that patient.

Even physicians with years of experience in diagnosing and treating anaphylaxis cannot determine, at the onset of an episode, whether that episode will remain mild or escalate over minutes to become life-threatening. It is therefore important that physicians instruct patients and caregivers to err on the side of prompt epinephrine injection.

Many patients and caregivers fail to carry EAs consistently or to use them when anaphylaxis occurs, even for severe symptoms, including throat tightness, difficulty breathing, wheezing, and loss of consciousness.

This authorship debate has raged since the 19th Century when clergyman James Wilmot set out to write a biography on the bard. This is substantiated by scholars who argue there is no real evidence to suggest otherwise.

Fellow writers, such as Christopher Marlowe and Ben Johnson, came from similar backgrounds to Shakespeare, so it seems entirely plausible that the son of a simple glove maker could be equally capable of high literary prowess.

Well, Anti-Stratfordians argue that, whilst genius is not dependent on social rank, access to knowledge and experience is. With a limited education, how does the boy from Stratford create works demonstrating an innate understanding of the royal courts, the geography of countries ranging from Scotland to Italy, politics and the law; not to mention a vast knowledge of classical literature?

Where would he get access to this information? Theories abound that this is when he accumulated the vast knowledge required to fill the plays with such minute detail.

Was he a soldier, a spy? Was he serving at court or simply trudging the country as a teacher or an actor? Or, is it more plausible that Shakespeare was someone else? Someone of higher rank who had access to the courts, travel and a higher level of education? Perhaps we will never know. In the meantime, just like a theatrical who-done-it, here are the top three un usual suspects who just may have written the plays:. Add in the high education, court position and penchant for travel and you have a hot prospect.

The theory was first proposed in by a Mr. T Looney yes, really and even garnered support from Sigmund Freud. A playwright, spy and contemporary of Shakespeare.

Shakespeare must have been a familiar figure at the royal court. Some of his plays, such as Macbeth , with its egregious flattery of King James I, bear obvious witness to the importance to the company of court patronage.

And the wide range of dramatic styles that Shakespeare adopted, the fact that he composed an average of around two plays a year, and that he moved freely among the dramatic genres, all indicate his sensitivity to the needs of his company.

The author is not as humble as he pretends to be — he keeps his options open. In this he differs greatly from, for instance, John Lyly, or even Ben Jonson, whose plays are more restricted in range. And though he knew that he had to please, he was willing, especially as he grew older - more confident of himself, less dependent on popular success - to push the boundaries, sometimes seeming almost to be writing for himself rather than for the populace.

Troilus and Cressida and Cymbeline , for example, are stylistically challenging; we have little evidence as to how these and some other plays fared with audiences. But it is clear from numerous references to Hamlet soon after composition that, for all its exceptional length, it, at least, was well received — which says a lot for the often under-rated audiences of the time.

Before Shakespeare even started to write a play he had to choose or to invent a story that was suitable for dramatization. Almost all his plays are based to some degree or other on one or more pre-existing narratives, some historical in origin, others fictional, some already in dramatic form. And he consulted some of these stories, especially the historical ones, in multiple versions. All this means that Shakespeare had to do a lot of preliminary spadework before he even began to invent a structure for his play.

He needed, and the company must have allowed him, time for reading. I find it irresistible to conjecture that they made it possible for him to move away from his London lodgings from time to time to the relative peace and quiet of a study — which we know existed - in New Place.

Some of the books Shakespeare had read at school clearly gave him material for both the stories and for details of his plays and poems. And it is clear that throughout his working life Shakespeare was an assiduous reader — and not only for professional reasons. All this shows that he was not just a jobbing playwright but a highly cultivated man of letters.

And he had to do all this in ways that would fit the physical structures of the theatres of his time and the strengths and limitations of the acting company at his disposal. He knew that the theatrical conventions of his time required plays to be of a certain length, though the limits were flexible. It is difficult for us to estimate how long the plays would have lasted in contemporary performance, but the fact that they vary in line length from around lines for The Comedy of Errors to about 4, for Hamlet shows that though there may have been minimum expectations there were no fixed limits.

Shakespeare may have wished to make changes between first having a manuscript transcribed for his actors and their putting it into production.

Play-texts had to be supple to local requirements. Shakespeare had models which he could observe, follow, modify, or reject. He knew about the traditional genres of tragedy and comedy, though he conspicuously refused to be constrained by them throughout his career. He wrote his plays as continuous structures, flowing smoothly from beginning to end.

He knew of the five-act structure favoured by Roman dramatists such as Plautus and Terence, and imitated by some of his English predecessors. But Shakespeare refused to be bound by the practice of his predecessors.

Sadly, little of the music composed for these purposes — or indeed for the plays in general - has survived. The printed texts contain music cues, but I suspect that they give an inadequate impression of the amount of music that would have been played in early performance. Most importantly, he would have to think about how his narrative material related to conventions of dramatic form and to expectations of genre, whether he could best relate it to conventions of comedy or of tragedy, or indeed whether it fell outside formal expectations of genre.

He would have needed to devise climactic scenes, and to think about how to bring it all to a satisfyingly dramatic conclusion. The ground plans for some of his plays are more schematically worked out than others. Other plays, however, such as The Comedy of Errors , Romeo and Juliet , and The Tempest , are elaborately and neatly plotted as if, like an architect designing a great cathedral, Shakespeare had created his overall design before going back to fill in the details.

And there is no way in which the intricacies of the virtuosically designed final scene of Cymbeline , with its multiple denouements, can have been improvised on the spur of the moment. Its composition required the same kind of intellectual effort as a contrapuntal masterpiece by Bach. And the conventions of comedy encouraged the inclusion of dances, of music and song.

Here he would have required the collaboration of composers, instrumentalists, and singing actors. For a tragedy Shakespeare would have wanted especially to give his actors the chance to portray passion, as Hamlet does, for example when he upbraids his mother, or Lear on the heath, or Othello in his jealous rage with Desdemona, or Coriolanus in his diatribes against the common people. But Shakespeare was not bound by convention, and his range and technique developed as he gained experience.

He broadened generic expectations. He would need constantly to remember how many actors he had available to him, and he would have to tailor the plot accordingly. Most of his plays can be performed by a company of 14 actors if, as was customary, some of them take more than one role each.

This seems sometimes to have placed strains upon his ingenuity. Alas, my liege, my wife is dead tonight. Shakespeare would want to write starring roles for the leading actors, and in doing so both to cater for their strengths and to remember their limitations. To say that Shakespeare would have to do all this is not, of course, to suggest that the writing of a play for the theatre of his time was any more difficult for him than for any of his fellow playwrights.

He was driven by internal compulsions, by changing and developing creative urges as well as by practical considerations. How fluent was Shakespeare? No working papers for any of the canonical plays survive.

Still, what they say is consonant with the evidence provided by his only surviving literary manuscript, the or so lines that he is believed to have added to the multi-authored play of Sir Thomas More after it had been subjected, at an uncertain date, to censorship by the Master of the Revels, Sir Thomas Buc. This is the only literary manuscript — indeed the only example of his handwriting except for a few signatures on legal documents - to have survived.

In it there is scarcely any punctuation, as if his ideas were flowing with such facility that he had no time to bother about details. Words are often abbreviated, to save time.

This is a man in a hurry, writing probably to commission to patch up a manuscript play after the Master of the Revels had demanded extensive change for political reasons. It is also possible to catch glimpses of Shakespeare at work in the printed texts of a few of his plays.

These rules are a little more rigorous: Callback contestants must prepare three current popular songs for their callback audition, and may be asked to perform a song of the producer's choice.

Unlike American Idol , original music and songs are not allowed because the show is primarily about the magic of hearing other voices interpret popular songs. While the first round didn't allow backing tracks or accompaniment, callback contestants may sing to a backing track, play an instrument, or have a person accompany them. In fact, a cappella singing is not allowed at the callbacks at all.

So with the right amount of force and hitting it at the right angle definitely causes the issue of having it come apart.

Similiar story with the sunroof exploding. Dustin: This is laminated. So laminated is a type of safety glass. Tempered, laminated, and then an acrylic or plexiglass, which is kind of a plastic base. So what we have here is two pieces of glass that are laminated together with a film- an adhesive film. And essentially this is annealed, or plate glass, on both sides. Dustin: So interesting fact: Almost everybody looks through this nearly every day because this is what your windshield is made out of.

But also we use this in a lot of areas for storefront. If you see a gas station that has been broken into, sometimes this is what we put in to deter thieves from getting in. We also use it in bullet-resistant glass. They laminate multiple pieces together. You can see where it broke from.

Swollen lymph nodes may also be present under the arm, near the collarbone, or both. It is important to note that these symptoms may also be signs of other diseases or conditions, such as an infection, injury, or another type of breast cancer that is locally advanced. For this reason, women with inflammatory breast cancer often have a delayed diagnosis of their disease. Inflammatory breast cancer can be difficult to diagnose. Often, there is no lump that can be felt during a physical exam or seen in a screening mammogram.

In addition, most women diagnosed with inflammatory breast cancer have dense breast tissue, which makes cancer detection in a screening mammogram more difficult. Also, because inflammatory breast cancer is so aggressive, it can arise between scheduled screening mammograms and progress quickly. The symptoms of inflammatory breast cancer may be mistaken for those of mastitis, which is an infection of the breast, or another form of locally advanced breast cancer.

To help prevent delays in diagnosis and in choosing the best course of treatment, an international panel of experts published guidelines on how doctors can diagnose and stage inflammatory breast cancer correctly. Their recommendations are summarized below. Proper diagnosis and staging of inflammatory breast cancer helps doctors develop the best treatment plan and estimate the likely outcome of the disease. Patients diagnosed with inflammatory breast cancer may want to consult a doctor who specializes in this disease.

Inflammatory breast cancer is generally treated first with systemic chemotherapy to help shrink the tumor, then with surgery to remove the tumor, followed by radiation therapy. This approach to treatment is called a multimodal approach.

Studies have found that women with inflammatory breast cancer who are treated with a multimodal approach have better responses to therapy and longer survival. Treatments used in a multimodal approach may include those described below. The prognosis, or likely outcome, for a patient diagnosed with cancer is often viewed as the chance that the cancer will be treated successfully and that the patient will recover completely.

Because inflammatory breast cancer usually develops quickly and spreads aggressively to other parts of the body, women diagnosed with this disease, in general, do not survive as long as women diagnosed with other types of breast cancer. Women who have inflammatory breast cancer are encouraged to talk with their doctor about their prognosis, given their particular situation.

Ongoing research, especially at the molecular level, will increase our understanding of how inflammatory breast cancer begins and progresses. This knowledge should enable the development of new treatments and more accurate prognoses for women diagnosed with this disease.

It is important, therefore, that women who are diagnosed with inflammatory breast cancer talk with their doctor about the option of participating in a clinical trial.
